Effective January 1, 2025, Local Form 5 should be filed using the new docket text entry event in CM/ECF.  This event does not require the uploading of a form and will simplify the process of submitting the declaration.  The new event can be found under the Bankruptcy Miscellaneous menu named Declaration in Support of Entry of Order - Docket Text Local Form 5.
